Definitions:
Service Animal: Any animal that aids his/her human partner or “master” with any of the disabilities covered by the ADA.
ADA: Americans with Disabilities Act
National Service Animal Registration Center: A low-cost Service animal registration service. (A.K.A. “National Service Animal and Trainer Registry” or “International Service Animal Registry”)
Trainer: The human primarily responsible for training the animal, i.e. the owner.
Human Teammate: Partner, “owner”, “master”, or human associate of the service animal
I agree and acknowledge that I am the primary trainer of my service animal and am responsible for my service animal’s training. This being true, I solemnly swear to keep my service animal up to date on vaccines, keep him/her healthy, well-exercised and loved, and to do my best to train my service animal to behave in polite manner and be mindful in public places. Furthermore, I agree that it is my responsibility to train my service animal for any and all tasks that help with my disabilities and/or the disabilities of any person with disabilities for whom I may be training the animal. Furthermore, I agree to comply with all state, county and municipal laws and regulations regarding service animals.
National Service Animal Registration Center, Paul Blair or any affiliates are not responsible in any way for any harm, damage or infractions the owner, handler, trainer or service animals may do.
